  • Acceptance (Date)
  • Date that the client's referral was accepted. Abbreviated to Acceptance in the Case List report.
  • Additional Referral Particulars
  • Additional details about the case.  The dropdown options  are end-user defineable through the Options Menu, Common Lists. 

  • Address
  • Postal address of the client.
  • Age
  • Age of the worker. Entries are end-user defineable through the Options Menu, Common Lists.
  • Approved Funds
  • Plan amount or estimate approved for the case.
  • Assigned To
  • Consultant who is responsible for the case.
  • Assigned To Email
  • Consultant's email address.
  • Assigned To Phone
  • Consultant's phone number.
  • Assigned To Qual
  • Consultant's qualifications.
  • Assigned To Title
  • Consultant's title e.g. Mr/Ms/Dr etc
  • Bill To (Company Name)
  • Company responsible for paying the invoices and abbreviated as the 'Bill To'.
  • Bill To Address
  • Postal address for the billing contact.
  • Bill To Email
  • Email address of the billing contact.
  • Bill To Fax
  • Fax number of the billing contact.
  • Bill To First Name
  • First name of the billing contact.
  • Bill To Full Name
  • Full name of thebilling contact (title, first and last names).
  • Bill To Last Name
  • Last name of the billing contact.
  • Bill To Mobile
  • Mobile phone number of the billing contact.
  • Bill To Notes
  • Notes stored  about the billing contact.  
  • Bill To Phone 1
  • Main contact phone number of the billing contact.
  • Bill To Phone 2
  • Alternative contact phone number for the billing contact.
  • Bill To Position
  • Work role of the billing contact.
  • Bill To Title
  • Title of the billing contact e.g. Mr/Ms/Dr etc.
  • Billed
  • Amount that has been currently invoiced for the case.
  • Biller Code
  • Biller code associated with the case.
  • Business Unit Name
  • An alternative field that can be used in lieu of the worker's name when invoices should not display the injured worker's name.  
  • Case Number
  • Case number automatically assigned by iinsight when a case is created.  This iinsight generated number can be modified by the user at Step 5 of the New Case Wizard.  Once assigned, the number cannot be changed.  
  • Case Status
  • Current status of the case, usually listed as Open, Closed, Reopened... etc. These dropdown entries are user-defined through the  Options Menu, Common list for "Case Status".
  • Cause
  • Nature of  injury or condition of the worker. The dropdown options for this field are end-user defineable through the Options Menu, Common Lists for "Cause".
  • Certified hours
  • Hours that the treating doctor has confirmed as available working hours.
